COST: $195ea (CCDFC club members $175ea.)

BOYS & GIRLS AGES: 7-10 camp time= 8-10am

BOYS 7 GIRLS AGES 11-14 camp time= 1030-1230pm

* PLEASE ARRIVE 30 MIN EARLY ON DAY 1 FOR CHECK-IN & ARRIVE 15 MIN EARLY DAYS 2&3 - PLEASE BRING CLEATS, SHIN GUARDS, & WATER *

Parent/Legal Guardian releases and holds harmless CC Dynasty FC, MBHS, and SLCUSD from loss, liability, claims, or damages whatsoever because of injury or death resulting from negligence or other acts to the minor. I hereby authorize the coaching staff to act on my behalf according to the best interest of my minor child and according to their best judgment should any injury occur. Photos may be taken and used to promote CCDFC and the Tristan Weber Soccer camp (i.e.. Social Media, web=site, media guide, etc.)
